Care Packages For Your Group

If you are requesting care packages for more than 3 people in the same unit, you are in the right place. Otherwise, please fill out the Individual Request Form.

Instructions for requesting care packages for your group:

1. Download the Request Form for Groups (Excel Spreadsheet)
2. Designate a Point of Contact (POC) for your group/unit
• the point of contact MUST be a military member
3. Fill it out and save it on your computer as an XLS, XLSX, or XLM file (Excel)
• name, rank, and email address for every military member are required
4. Go to the upload form below
5. Follow all instructions on the form
6. Upload your saved XLS, XLSX, or XLM Request Form
7. Verify that you have entered all required information correctly
8. Click the Upload Care Package Request Button one time only

If you do not have the information required for requesting packages for a group, please use the individual request form.

If you can't follow SIMPLE instructions on this form SKIP WILL NOT send care packages to the group you're requesting packages for. READ AND FOLLOW ALL INSTRUCTIONS.
